The IndyMed Preparedness Initiative:
A roadmap from Corona, Long COVID, Post-Vac to Chronic Fatigue Syndrome and P4 Family Medicine

Abstract

The IndyMed initiative presents a roadmap of scientific milestones in the management of the Corona-pandemic by comparing COVID-19 fatality rates in Germany 2020 (no vaccination), 2021 (vaccination) and 2022 (SARS-CoV-2-Omicron). The influence of social media (talk shows) and of expert groups representing different medical disciplines are put in perspective to published trans-/inter-/disciplinary scientific knowledge in virology, in immunology and genetics by highlighting: I. Babylonian Confusion: Different fatality rates of SARS-CoV-2 Delta vs. Omicron turn out to be vaccination independent. II. COVID-19 is a polygenic disease evinced by whole genome SNP risk determination. III. ME/CFS is hypothesized to be an autoimmune process, mediated by IgG trafficking / MHC allele specific peptide presentation in concert with a second trigger of inflammatory nature. IV. A Preparedness Plan outlines steps how agent-based systems enable more personalized NPI healthcare decisions in pandemic scenarios. Finally, the diagnostic power of lineal family based transgenerational genetics is illustrated. Artificial intelligence (AI) driven knowledge based expert systems are projected to determine predictive health related SNPs based on Direct to Consumer (DTC) data to be stored on personal Electronic Health Records. Legal and ethical aspects of P4 Family Medicine approaches demand the implementation of a Family Medicine Privacy Amendment (FMPA).
